What to Expect During the Intake Process

Step 1: Initial Contact

Your journey begins with reaching out to us via phone, email, or our online form. During this initial contact, our friendly team will gather some basic information and answer any questions you might have about our programs or services. We’ll also schedule a time for your intake appointment.

Step 2: Comprehensive Assessment

During your intake appointment, you’ll sit down with one of our licensed clinicians for a personal assessment.  This step allows us to understand your unique challenges, history, and goals. 

Here’s what our assessment covers:

  • Medical History: A review of your physical health and any medications you’re currently taking.
  • Mental Health History: A discussion about any past or current mental health challenges.
  • Substance Use History (If Applicable): Understanding patterns, triggers, and past treatment efforts.
  • Personal Goals: Identifying what you hope to achieve during treatment.

This detailed assessment helps us figure out the best level of care for you, whether that’s PHP, IOP, or a partial care program.

Step 3: Insurance Verification and Financial Planning

We’ll handle the details of verifying your insurance coverage and make sure to explain any financial responsibilities right from the start. Our team will closely work with you to keep everything transparent and help you understand your options.

Step 4: Personalized Treatment Plan

Based on the results of your assessment, we’ll create a preliminary treatment plan that’s tailored just for you. Your plan may include a combination of evidence-based therapies, medication management, group sessions, and additional resources to support your recovery journey.

Step 5: Orientation

To help you feel prepared and confident, we will provide a quick orientation to familiarize you with our facility, schedule, and team. We’ll also go over your treatment plan and talk about how we plan to achieve your goals. 

Once all these steps are complete, our team will finalize your intake, and you’ll officially begin your healing journey with us.

What to Bring With You for Your Intake

Being prepared can really make things go smoothly and efficiently during your intake appointment. Here’s a list of items to bring with you when you arrive:

  • Photo Identification: You’ll need a valid driver’s license, passport, or any other government-issued ID for photo identification.
  • Insurance Information: If you have insurance that will cover any part of your treatment, please bring your insurance card and any related paperwork.
  • List of Current Medications: Include the names, dosages, and frequencies of any medications you are currently taking. If you can, please bring the actual medication containers so we can get the right information.
  • Previous Medical Records (If Applicable): Bring any relevant medical records from previous treatments or interventions. They can really help our team get a better understanding of your situation.
  • Emergency Contact Information: Provide the details of a close family member or friend we can contact in case of emergencies.
